What Can ECM Do for You?
When you qualify for our ECM program, you’ll be paired with a community health worker (CHW) — your very own healthcare advocate! Your CHW will be there to support you and collaborate with you and your healthcare team to ensure your care is coordinated and aligned with your unique needs.
